All Indiana University Ï㽶ÊÓƵs are required by state law to present proof of vaccination against various diseases. You should submit your compliance form before you begin classes. You will not be able to register for your second semester until you have done so.
All evidence you submit must be in English.

If you will be under the age of 18 when you arrive on campus, your parent or guardian must complete a form in order to have your TB blood test.
If you receive any vaccinations at IU while on the , the cost will be covered.
As part of the arrival process, all international undergraduate and graduate Ï㽶ÊÓƵs are required by Indiana law to take a tuberculosis (TB) test with the Student Health Center. The online scheduler is open now, and we encourage you to sign up as soon as possible so you can complete your required testing on time.
International Ï㽶ÊÓƵs can schedule their TB test appointment via and choosing the following after logging in with their IU credentials:

You will need your IU International Insurance card to make an appointment with the IU Health Center. If you have already enrolled in classes at IU, you will be able to access your insurance cards through Anthem’s website starting August 1. Create your to look up your Anthem ID card. Make sure to enter your name and date of birth exactly as it is displayed on your passport.

Please do not bring TB documentation from outside of the United States; it will not be accepted.
